This form must be filled out, signed and in the Louisiana CheerNastics office to complete your birthday party booking. You can fax this form to 798-0684. Our office hours are 2pm to 7pm Monday through Friday. Once we have received this form and your deposit your party spot will be held on our schedule.
We must have this form in our office for registration to be complete. The first 100 Registrations received will receive a FREE T-shirt!
This flyer is available to anyone wanting to help us promote this exciting event at their gym, school, etc.
This schedule is TENTATIVE. It is subject to change if competition companies change dates, locations or prices. If there are changes in the schedule all parents will be notified by email and a edited competition schedule will be posted on this web-site. La. Rebel Hotel Blocks will be posted the week of July 22nd for all out of town competitions. We are excited about the schedule this year and are looking forward to a great season! Let's GO REBELS!!!! This form is requireded in our office to withdraw and/or stop payment on a tumbling class. The form is to be faxed, emailed to email@example.com or dropped by our office no later than the 20th day of the month prior to cancellation to allow LCN a chance to fill the spot before the next month. Any withdrawals received after the 20th day of the month will be charged for the next month tuition as stated in the Tumbling Class Registration Packet. Please call our office for any further questions.
Required for all non-members who are attending a Flip Flop Friday, Cheer Camp, Cheer Clinic, or Open Gym/Skill Reinforcement event.
