This form must be completed and turned in to the office along with a $75.00 non-refundable/non-transferrable deposit for your booking to be complete.

This form is requireded in our office to withdraw and/or stop payment on a tumbling class. The form is to be faxed, emailed to firstname.lastname@example.org or dropped by our office no later than the 20th day of the month prior to cancellation to allow LCN a chance to fill the spot before the next month. Any withdrawals received after the 20th day of the month will be charged for the next month tuition as stated in the Tumbling Class Registration Packet. Please call our office for any further questions.
These answers are to many questions that we receive each year concerning our placements in our Rebel Team program. Many of these answers DO apply to our Show Team program as well but we do recognize the amount of time each athlete has had in the sport/activity. Ex. We'll not evaluate a 5 year old first year the way we evaluate an 8 year old in their 2nd or third year.
