This form must be completed and turned in to the office along with a $75.00 non-refundable/non-transferrable deposit for your booking to be complete.

This form is required to attend our 4th Annual leadership rally! This is a great event and very beneficial for health, well-being and development of teenagers! ALL are welcome to attend!
This schedule is subject to change if necessary. If so, parents and athletes will be notified immediately. Please note the Hotels that have the "Stay to Play Policy". You must book through the link listed for your athlete to be eligible to compete at those events.

This form is requireded in our office to withdraw and/or stop payment on a tumbling class. The form is to be faxed, emailed to email@example.com or dropped by our office no later than the 20th day of the month prior to cancellation to allow LCN a chance to fill the spot before the next month. Any withdrawals received after the 20th day of the month will be charged for the next month tuition as stated in the Tumbling Class Registration Packet.
